Diagnostic Evaluation

During your first new patient visit, your doctor will begin determining the exact cause of your foot pain so that a course of treatment can be made. Some of the diagnostic tests Dr. Arabshahi may perform include:
  • Disease Screenings
  •  Ankle Brachial Index (ABI) for Peripheral Arterial Disease screening (PAD)
  • Quantitative Sudomotor Axon Reflex Test (QSART) for Neuropathy
  • Ultrasound

Common Foot Ailments

  • Bunions:  This bony bump on the outside edge of your big toe is a form of arthritis. It indicates that degenerative changes are taking place in the bone beneath. In fact, the bunion may be causing your first toe to push inward against your second toe and create a hammertoe. Not all bunions result in complete swinging-in of the big toe. There are simple bunions that appear on the side of a straight big toe and others on the top of the big toe joint, (as opposed to the side.)   Only surgery can clear up your problem once and for all. There are several different surgical procedures, and in most cases they are done on an ambulatory basis. There is no need for hospitalization for the average person.   Talk to Dr. Arabshahi to discuss different surgery options.
  • Corns:  A corn is a hard, thickened area of skin found on the top of, the tip of, or between your toes. It's round and yellow. If yours is reddish, you've got an inflamed corn. A corn has a central core that descends into your flesh in a cone-shaped point killing all the normal cells in its way.  Chronic corns are caused by a hammertoe condition. Recurring corns can be eliminated through a minor surgical procedure. Additionally, collagen treatment brings relief to some corn sufferers and is the treatment of choice for those who are unable to undergo surgery.
  • Calluses:  Layers of dead, thickened skin that also form to protect bones and tissue. If you are on you feet a lot, you may need additional cushioning.  To reduce calluses, buff calluses with a pumice stone or brush. Apply moisturizer and wear socks to bed.  If calluses are caused by structural foot problems, you may need custom orthotics. Sore calluses are almost always the result of these problems, and they won't go away until the underlying problems are corrected. 
  • Flat Feet & Heel pain: Heel spurs and plantar fasciitis (inflammation of the ligament on the bottom of the foot which attaches to the heel and maintains our arch) are the most common causes of heel pain. These problems usually occur in pronated feet (feet that roll inward, breaking down the inner sides of shoes). Pronation forces the heel to strike the ground abnormally, and places abnormal stress on the heel and plantar fascia which causes pain that will become progressively worse. Your pain may occur only when first standing, and then disappear; it may occur at irregular intervals; or it may be constant. In any case, be assured, it will become worse without custom-made orthotics. The custom-made orthotic is constructed to: allow the heel to strike the ground in the correct manner by eliminating pronation; cushion the heel; and support the arch. Custom-made orthotics are the first and most successful line of treatment podiatrists use for these problems.  Diode laser are extremely effective and you may see reliefe with just 3-6 sessions of laser.  Orthotripsy has also shown to be very effective although not all insurnaces cover this non-invasive treatment option.
  • Hammertoe:  Hammertoe is a permanent deformity of one of your middle toes-probably the big toe's neighbor-in which the joint has become bent up and twisted.  Correcting a flexible hammertoe is classified as minor surgery.  Dr. Arabshahi maybe able to do a minimal incision surgery with less recovery time.
  • Ingrown Toenails:  Ingrown toenails are painful so we'll get you in right away!  Ingrown toenails can be extremely painful. In addition, if they are not treated early, an infection can often set in.   Thus, we make sure that patients with ingrown toenails are seen right away.  Ingrown toenails usually resolve very quickly after the ingrown portion of the nail is removed. 
  • Neuroma:   If you have severe pain, or electric shock type pain, or numbness, or tingling, and often feel like you have to take off your shoe and rub bottom of your foot? You may have a "neuroma." This is a swelling of a nerve and you should come in for evaluation and treatment. Relief might only be only a few treatments away.  Diode laser could be a very effective treatment to traditional treatment.
  • Wart:   Warts are caused by a virus and are highly contagious and occur at the bottom of the foot. Recurrence is quite common. Plantar warts are very painful when walking and can sometimes be mistaken as calluses.  Warts may be treated by chemical removal, or surgical curettage.
